Description
Group visits offer a tremendous opportunity to educate and empower patients to implement lifestyle behavior changes that support improved health outcomes. They are also an efficient use of provider resources as they allow similar interventions to be scaled beyond a one-on-one office visit. Group dynamics that include social support and knowledge sharing also enable increased levels of patient self-empowerment and efficacy. In this session, you will hear from seasoned healthcare providers who are effectively leveraging group visits to achieve better outcomes among diverse patient populations in various healthcare settings.

Columbus-BatisteColumbus D. Batiste, II, MD, FACC, FSCAI | Medical Director - Home Based Cardiac Rehab Program, Kaiser Permanente

Dr. Columbus D. Batiste is an Interventional Cardiologist, Assistant Clinical Professor at the University of California Riverside School of Medicine, former Chief of Cardiology for 12 years and current Regional Medical Director of the KP Home Based Cardiac Rehab program . In 2010, Dr. Batiste sought to break-the-cycle of prescriptions and procedures as the sole management of chronic disease and began promoting a long-term solution for his patients through nutrition, stress reduction, and exercise. As a result, in 2011 Dr. Batiste established a lifestyle intervention program entitled the Integrative Cardiovascular Disease Program (based at Kaiser Permanente). In 2016 Dr. Batiste led a group that collaborated with Samsung Technologies and developed a virtual cardiac rehabilitation program utilizing a Samsung wearable. Since its launch, the program, which applies the principles of lifestyle, has treated nearly 10,000 patients. Dr. Batiste’s mission and his passion for the community has led to the formation of a non-profit organization called the Healthy Heart Nation, which provides education through lectures, newsletters, social and digital media.

Beth-FratesBeth Frates, MD, FACLM, DipABLM | Harvard Medical School Faculty, Harvard Medical School 

Beth Frates, MD, is trained as a physiatrist and a health and wellness coach. She served as a member of the Board of Directors of ACLM from 2016-2019 and was voted President-Elect in 2020. She was 1 of 4 item writers for the ABLM. She is an award-winning teacher at Harvard Medical School, where she is assistant clinical professor. She developed and taught a college lifestyle medicine curriculum at the Harvard Extension School and that lifestyle medicine syllabus, which can be downloaded through the ACLM website, in hopes that her work can serve as a template for other instructors and professors. Along with colleagues and staff at ACLM, she created the LM 101 Curriculum, which can be downloaded through ACLM website. Dr. Frates co-authored the Lifestyle Medicine Handbook published in 2018 by Healthy Learning, which is ranked 51 out of the top 100 medical books ever from Book Authority. In addition, she created and implemented a group lifestyle medicine wellness program called PAVING the Path to Wellness, which she now delivers virtually. Most recently, Dr. Frates was recruited to be the first Director of Lifestyle Medicine and Wellness for the Department of Surgery at MGH.

Jeff-GellarJeff Gellar, MD Director of Integrative Medicine , Kronos Health; President, Integrated Center for Group Medical Visit; Co-Founder, Integrative Medicine for the Underserved (IM4US)

Jeffrey Geller, MD, the 2020 Massachusetts Family Physician of the year, is known for his innovations in primary health care. Since graduating Tufts Medical School in 1996, he developed the modern group medical visit (GMV) models and protocols with the primary goal of treating loneliness. His work with open models which prioritize human connection are a major influence in the field. By pairing integrative treatments with GMV such as Tai Chi, massage, exercise, acupuncture, yoga, and hypnosis he had found an effective method of providing non-pharmacologic treatment for chronic pain, obesity, chronic illness, and addiction. He has won numerous awards from the U.S. Surgeon General, Congress, AAFP, IM4US, and FMEC for creating the largest U.S. group medical visit program which helped patients overcome barriers to health. He helped found Integrative Medicine for the Underserved (IM4US) and is president of the non-profit Integrated Center for Group Medical Visits (ICGMV). ICGMV has the goal of innovating, training, and supporting, the GMV community as well as evaluating major GMV concepts such as the Group Inclusion Effect (GIE). Dr. Geller believes that ICGMV, in collaboration with Kronos Health, will revolutionize primary healthcare - giving patients, providers, and communities the time and support they need to create health.

Jacob-MirskyJacob Mirsky, MD, MA | Primary Care Physician, Massachusetts General Hospital

Jacob Mirsky MD MA is a primary care physician at the Massachusetts General Hospital Revere HealthCare Center and a consultation physician at the Benson-Henry Institute for Mind Body Medicine. Dr. Mirsky attended medical school at the University of California San Francisco and completed residency in internal medicine at the Brigham and Women’s Hospital in Boston as part of the Harvard Vanguard Medical Associates primary care track. Dr. Mirsky views patient education and empowerment as the cornerstone of health. In addition to his primary care practice, he leads group visits (also called shared medical appointments) in primary care focused on healthy lifestyle change for symptom management and disease prevention. He helped open the MGH Revere Food Pantry in 2019, which provides free plant-based foods to patients with food insecurity, and he currently serves as its Medical Director. Dr. Mirsky also founded and is the Director of Clinical Innovation of the Division of General Internal Medicine Healthy Lifestyle Program (HLP). The HLP has a mission to establish healthy lifestyle change as the standard of care for preventing and treating chronic disease.

Format
LM2021 OnDemand sessions are hosted online via the ACLM Learning Management System and each session consists of a title, description, learning objectives, speaker information, video recording, presentation slides in PDF format, an assessment and survey evaluation.
