Description
Behavior change is the foundation for effective lifestyle prescriptions, yet the path to lasting lifestyle change is complex, individualized and influenced by a multitude of factors. As such, it extends beyond solely prescribing and educating patients on what to do, and necessitates cultivating the practical skills and systems by which to empower patients to develop and sustain health-promoting behaviors. This session aims to equip clinicians with evidence-based skills, strategies and practical frameworks to support behavior change within brief clinical visits. Additionally, attendees will examine best practices and key considerations for effective interprofessional collaboration at a systems level, to include real world examples of the successful integration of health and wellness coaching within health systems.

Jasmol-SardanaJasmol Sardana, DO, DipABLM | Physician, Barnard Medical Center

Dr. Jasmol Sardana is a board-certified internal medicine physician and newly board-certified lifestyle medicine physician who is equally passionate about lifestyle medicine, culinary medicine and community service. The importance of giving back to her community was impressed upon Dr. Sardana at a young age when she began volunteering with the homeless communities of Washington DC, through Martha's Table, a local charity. There, Dr. Sardana saw first-hand the direct effect that access to nutritious food can have on marginalized communities which left a lasting impact on her. As the daughter of first-generation immigrants, who once had to rely on governmental assistance, she has personally experienced the socioeconomic burdens and the effects of limited access to resources in an often difficult-to-navigate system. Which is why serving similarly underserved communities has been a priority for her. Dr. Sardana received her medical education from Nova Southeastern University's College of Osteopathic Medicine, where the osteopathic philosophy of treating the whole person has resonated with her throughout her medical career. Her sense of responsibility to the community deepened in medical school where she sought out and coordinated volunteer opportunities for herself and fellow medical students leading to her induction into the Honorary Osteopathic Service Fraternity, Sigma Sigma Phi. Dr. Sardana founded and currently chairs the Community Outreach Committee at the American College of Lifestyle Medicine, whose focus is to extend ACLM's mission of "helping individuals and families adopt healthy behaviors" to those in need by empowering, encouraging and educating ACLM members to utilize the vehicles of volunteerism and community engagement to achieve health equity in their own communities.

Jessica MatthewsJessica Matthews, DBH, NBC-HWC, DipACLM | Assistant Professor and Program Director - Kinesiology in Integrative Wellness, Point Loma Nazarene University; Director - Integrative Health Coaching, UC San Diego

Dr. Jessica Matthews, DBH, NBC-HWC, DipACLM, is assistant professor and director of the Master of Kinesiology in Integrative Wellness program at Point Loma Nazarene University. An award-winning and innovative educator, she has created and implemented graduate degree and professional certificate programs at several colleges in Southern California, in addition to teaching a variety of graduate courses in areas such as lifestyle medicine and integrative health. A national board-certified health and wellness coach (NBC-HWC) and board member of the National Board for Health and Wellness Coaching (NBHWC), Dr. Matthews serves as director of integrative health coaching in the Centers for Integrative Health at UC San Diego in the Department of Family Medicine and Public Health. She also serves as founding chair of the ACLM health and wellness coaches member interest group and is lead faculty in the area of health behavior change for the Lifestyle Medicine Residency Curriculum. A trusted resource for evidence-based wellness information, for over a decade Dr. Matthews has regularly contributed to numerous media outlets, including, but not limited to, CNN, The Washington Post, Women’s Health, The New York Times, NPR, and Doctor Radio. She has authored and edited several books, including The Professional’s Guide to Health and Wellness Coaching, and in 2019 delivered a TEDx talk aptly titled “Reclaiming Wellness.” Dr. Matthews is genuinely passionate about inspiring others to thrive in their lives—physically, mentally, emotionally, spiritually and socially—while simultaneously serving the greater good. As such, for years she led donation-based oceanfront yoga classes in which 100% of proceeds were donated to various non-profit organizations which speak to her heart. For her superior instructional abilities and positive influence within her local community and the global health and fitness industry as a yoga educator, Dr. Matthews was honored as the 2017 IDEA Fitness Instructor of the Year.

LM2021 OnDemand sessions are hosted online via the ACLM Learning Management System and each session consists of a title, description, learning objectives, speaker information, video recording, presentation slides in PDF format, an assessment and survey evaluation.
